FSSAI documents checklist, by licence type

The documents FoSCoS asks for on a Registration, a State Licence and a Central Licence — and the three that hold most applications up.

A set of documents and a checklist

FSSAI Registration (up to ₹1.5 crore)

  1. Photograph of the proprietor
  2. Photo ID — Aadhaar, PAN, voter ID or passport
  3. Proof of premises — rent agreement, ownership papers or NOC from the owner
  4. List of the food items you make or sell

State Licence (₹1.5–50 crore)

  1. Form B, signed by the proprietor, a partner or a director
  2. Photo ID and address proof of the proprietor, partners or directors
  3. Proof of possession of the premises
  4. Constitution of the business — partnership deed, MoA/AoA or incorporation certificate
  5. List of food categories you will make, store or sell
  6. Food Safety Management System (FSMS) plan
  7. Nomination of a responsible person (Form IX, for companies)

Manufacturers add:

  1. Layout plan of the processing area, with dimensions
  2. List of equipment and machinery, with capacity
  3. Water test report from an FSSAI-notified or NABL lab, where water goes into the food

Central Licence

Everything on the State list, plus where it applies:

  1. Importer-Exporter Code (IEC) from DGFT, for import and export
  2. Star-classification certificate from the Ministry of Tourism, for hotels
  3. List of premises by state, for a head-office licence

Three things that commonly cause queries

  • A kind of business that doesn’t match the documents. The application asks what you do; the layout plan, equipment list and product list have to say the same thing.
  • A water report from the wrong lab. It needs to come from an FSSAI-notified or NABL-accredited laboratory, and be recent.
  • Premises proof that doesn’t match the address. The rent agreement or ownership document has to name the same premises the licence will cover.

A note on these lists
