1. Do you hold an FSSAI licence or registration for every premises you operate?
2. Does the kind of business on it describe what you do today?
3. Has the annual fee been paid for the current year?
4. Do you manufacture or import food?
5. If you manufacture or import: was Form D-1 filed by 31 May for last year?
6. Did your licence expire before 10 March 2026 without being renewed?
7. Has your turnover crossed ₹1.5 crore or ₹50 crore since you applied?
